Planning to specialize in HF?


The American Board of Medical Specialties recently approved the American Board of Internal Medicine’s proposal to establish secondary subspecialty board certification in advanced heart failure and transplant cardiology. Cleveland Clinic cardiology fellow Dr Eiran Gorodeski interviews Dr Marvin Konstam on the implications of this move: what does it mean for fellows heading into HF and for physicians who already specialize in HF?

I am wondering if you know of any fellowship training for non physicians or midlevel providers. We have recently started a heart failure clinic at our clinic and I would like to pursue some advanced training.

Sub specialty board certification in advanced heart failure and transplant cardiology,  is a huge step forward in the management of heart failure which is becoming increasingly prevalent in the western world as well as third world countries.

So many physicians care for heart failure patients & run  CHF clinics in the third world, giving recognition to those physicians is very important , based on their experiences, medical degrees & possibly allowing them to take Sub specialty board certification exam in advanced heart failure and transplant cardiology.

I hope it will be allowed for physicians & cardiologists from outside US .

In my hospital our cardiology staff run very effective CHF clinics, hundreds of patients with systolic or diastolic heart failure are enrolled, the staff which include general cardiologists & very importantly dedicated CHF specialist nurses with prior CCU experience share together the pharmacological & non-pharmacological management of heart failure patients, every effort is made to make sure that those patients are on optimum doses of ACE I or ARBs plus Beta blockers,salt restriction & exercise program, the CHF clinics staff are up to date with all the details which is related to the management of HF including the very latest HF trials.

The staff refer patients for devise insertion when it's due or for transplant & provide post- devise or transplant followup .

The result was a  significant reduction in cardiology admissions, i hope the staff will be certified with this new subspecialty board .

In our counrty (Egypt), the prevalence of heart failure is high ,although we have more cases of end satge valvular heart diseases as an etiology of HF ,in a previous study done by myself and othr colleauges at the cardiology department, cairo university ,we found that the common etiology as in western is CAD .With aging and proper management of myocardial infarction , the incidence of HF is increasing . Unfortunately, we dont have cardiac transplantation at our country ,also we dont have assist devices .So, we are using maximal medical treatment and this depend on the experience of cardiologists. for this reason , specialists in traetment of heart failure is ugently needed .i have special interest in this field and on 1999, i joined dr Lynne stevenson at the brigham and womens hospital ,one of the experts in managing end stage HF and i learned from here how to manage heart failure patients who are critically ill .

Heart failure clinics are important for proper management of heart failure patients as an outpatient ,but in our country we dont have the nursing team which play an imporant role in management of those patients .
